Correction of Serum Fructosamine Concentration in Hypoalbuminemia




Abstract
Serum fructosamine concentration reflects the time-integrated glucose concentration over the previous one to three weeks and can be used for the follow up of diabetes mellitus(DM). Some investigators have reported that the fructosamine concentration was influenced by the albumin level. This study attempted to figure out the correction equation for fructosamine concentration in hypoalbuminemia.
Serum fructosamine was measured in 75 diabetics; 32 non-diabetic hospitalized patients and 38 normal control. The serum fructosamine concentration was 2.56¡¾0.36 mmol/L (mean+ 2SD) in normal control(n=38), 2.50 mmol/L(SD 0.37) in non-DM without hypoalbuminemia (n = 23), 3. 50 mmol/L (SD 0. 62) in DM patients without
hypoalbuminemia (n = 65) , 2.74 mmol/L (SD 0.57) in DM patients with hypoalbuminemia (n =10), and 2.14 mmol/L (SD 0.42) in non-DM paitents with hypoalbuminemia (n = 9). The significant difference in fructosamine levels was observed between normal group and DM without hypoalbuminemia(p<0.001), however, no difference between normal group and DM with hypoalbuminemia.
The correction equation for the influence of hypoalbuminemia on fructosamine level was as followed.

In DM patient with hypoalbuminemia, corrected fructosamine was $.14¡¾0.57 mmol/ L (mean¡¾SD) , which was significantly higher than that of normal group. (p<0.001).
